I have seen a physio about my current patellofemoral issues. They suggest much of what you have, however I’ve not seen an improvement over 3 months. Little pain during exercise increasing after and next day. Feels angry if that makes sense. Is it just a cause of doing too much? But if I back off I feel like I’m not going to strengthen the quad and fix the problem. Ideas? Thanks

No pain, just progressive dull aching if sitting in a chair for a long time but the main issue is painless inflammation of the fat pad and plica as my patella tendon impinges on the femoral condyle giving me patella tendon friction compression syndrome as well. How do I fix this as nothing has worked, from Ben Patrick’s Zero Workout, to using multiple physios and doing their lower body work. I just can’t seem to figure it out. Why doesn’t my inflammation heal? What am I doing wrong? I believe it is due to exercising every day but I know it’s a systematic pattern problem with my knees or it wouldn’t be long term. So my q is simply what do I need to do to heal the inflamed tissue? 1) Rest for long enough and then start small progressions 2) Keep doing my exercises and trust the process to make my muscles strong enough to gradually deload the tissues 3) Get surgery for the poor structural mechanics I have (as shown on bilateral mri). Do I need initial long period of rest?

Why do three at all? One set and reps in reserve keeps patients safe from overdosing and creating immune reaction and acute on chronic pain again. One pain free rep is better than a third set that flares them up.
